As a Therapist within the Home First Team you will be involved in many aspects of a patient’s journey from hospital to home. Our team support patients: within the Emergency Village (EV) where they need additional support to return home; through the triage of patient referrals referral to the service with MDT discussion around what pathway is suited to meet their needs and promote independence; and in patient’s homes or short-term placements to help them achieve their rehabilitation goals. This role is primarily based within the community, however there is flexibility in the location of assessments as the service continues to develop, so patients may be assessed for home in the hospital, at home, or within the Emergency Village in Salford Royal.



Home First utilises a Strengths Based Approach in its assessment and intervention with patients, and our Multi-Disciplinary Team is made up of therapists including technicians, nurses, social workers, pharmacists, housing workers, reablement support workers, assistant practitioners, and care coordinators.

We work closely with patients, families and partner organisations and liaise with and signpost to non-statutory services where identified and appropriate.

To support and facilitate hospital discharges within a timely manner providing appropriate assessments.

To support with the triage process and participate and at times lead discussion around the discharge destinations of patients who are in hospital, or those in the community who may need a short period of rehabilitation and reablement.

To assess and treat people in their own homes who are experiencing reduced physical and cognitive function with the goal to support them in returning to a level of independence following a functional deficit.

To provide advice on the management of these patients to any Health and Social Care Professional working within other services, this includes appropriate placement of patients within the community.

To support the wider team and assist in the development of services within the MDT environment. Therefore, the ability to work as part of a team, leadership skills and experience of supervising staff and students are part of the essential criterion.

To provide a Therapy service to people in their own homes across Salford over a 7-day working pattern.
