Job Description Post Title: Community Nurse Department: Community Learning Disability Nursing Location: Wolverhampton Directorate: Learning Disabilities Band: 5 Hours: full time (37.5hrs pw Mon-Fri) Contract Type: Permanent Responsible to: Community Nurse Manager Job Purpose To provide specialist support, advice and guidance to individuals, families and carers. To provide specialist support as part of the multi-disciplinary team To complete specialist assessments as clinically indicated To participate in PBS training and offer specialist support within the PBS framework To participate in referral meeting, team meetings and wider MDT meetings. To contribute to team and service development. To carry an individual caseload. To keep up-to-date with research and development in your fields of practice To attend training relevant to the role To maintain NMC registration and remain up-to-date with research and development in learning disability nursing practice To complete individual risk assessments under supervision and to put a safety plan in place if needed. To communicate with social workers, care providers and other agencies and services. To supervise and assess student nurses on placement with the team.
